QUAD is MEDVi’s compounded “4-in-1” sublingual erectile dysfunction product, from $119 a month with a consultation included and, the company says, a 15-minute onset and a 36-hour window. Four active ingredients sounds like more medicine than one. In this case that is the problem rather than the selling point.
Three Drugs, One Mechanism
Independent sources report QUAD as combining sildenafil, tadalafil and vardenafil — the active ingredients of Viagra, Cialis and Levitra — plus apomorphine. Those first three are all PDE5 inhibitors. They work by the same mechanism, on the same enzyme.
Combining drugs from different classes can be sound medicine, because they act on different targets. Combining three drugs from the same class does not add a new mechanism. It adds dose. You are not getting a cleverer treatment; you are getting more PDE5 inhibition, delivered by three molecules instead of one, with no trial evidence that this is better than an adequate dose of any single one.
The pharmacokinetics make it worse rather than better. Sildenafil and vardenafil are relatively short-acting, at roughly four to five hours. Tadalafil has a half-life around 17.5 hours, which is where the “36-hour window” claim comes from. Stacking them means a sharp early peak layered on top of a long tail — so if you do experience side effects such as headache, flushing, nasal congestion, visual disturbance or a drop in blood pressure, you cannot simply wait out a short-acting drug. Part of the dose is still with you the next day.
The hard contraindication is unchanged and is worth stating plainly: PDE5 inhibitors must never be combined with nitrates — nitroglycerin, isosorbide and related drugs — because the blood pressure drop can be fatal. Caution also applies with alpha-blockers, commonly prescribed for blood pressure and prostate symptoms. A higher combined PDE5 load does not make that safer.
Apomorphine is the fourth component in that reported formulation, and it is a different mechanism — a centrally-acting dopamine agonist. It was developed for ED as Uprima and did not establish itself in the US market. Nausea and syncope are recognised effects. The alternative formulation some sources describe replaces it with PT-141 (bremelanotide), which is approved in the US only for hypoactive sexual desire disorder in premenopausal women, and would be off-label here.
Nobody Agrees What Is In It
This is its own finding. Some sources describe QUAD as sildenafil, tadalafil, vardenafil and apomorphine. Others describe sildenafil, tadalafil, PT-141 and L-citrulline. Those are materially different products with different risk profiles, and one of them contains an amino acid supplement where the other contains a third prescription PDE5 inhibitor.
Some of that may be genuine variation — compounded preparations can differ by pharmacy and by version, which is one of the standing objections to compounded products generally. But for a prescription drug combination, the buyer needs to know exactly which molecules and which doses they are being sold, before the first dose rather than after.
Get the formulation and per-ingredient doses in writing. If you take any other medication — particularly anything cardiovascular — take that written list to a pharmacist before you take a dose. This is the same argument we make about undisclosed doses in supplements, except the stakes are considerably higher when three of the ingredients are prescription vasodilators.
The “Independent” Reviews Are Not
A practical warning about researching this product. Searching for independent analysis of QUAD returns a large number of pages presenting themselves as clinical or medical-centre reviews, on domains that look like hospitals, urology practices or research consortia. Several are marketing content, and at least one prominent result is published by a direct competitor.

A page that reads like a medical centre and is actually a competitor selling you a rival ED product is a different thing. The company’s cited 4.9-star rating is also worth treating carefully — we could not verify it at source, and this is a category where review quality is unusually hard to assess.
ED Is Often the Symptom, Not the Problem
A 2023 meta-analysis found GLP-1 receptor agonists significantly improved International Index of Erectile Function scores versus placebo, with the greatest improvements in men with the highest baseline BMI.
An ENDO 2025 study of 83 men with obesity and erectile dysfunction compared tirzepatide against transdermal testosterone — and tirzepatide produced better erectile function scores, despite not being a hormone treatment at all.
The reason is mechanical rather than mysterious. An erection is a vascular event, dependent on healthy endothelium — the lining of the blood vessels. Obesity drives endothelial dysfunction through inflammation, oxidative stress, raised blood pressure and insulin resistance. Treat that, and erectile function improves through the same pathway that improves everything else.
There is a hormonal route as well. Adipose tissue is rich in aromatase, which converts testosterone to estrogen, and the resulting estrogen suppresses the pituitary signal that tells the testes to make more. Weight loss of at least 10% has been associated with testosterone rises of roughly 55 to 115 ng/dL in clinical trials — covered further in our TRT review.
None of which means a PDE5 inhibitor is a bad idea. They work, they are well characterised, and nobody should be told to lose 30 pounds before they are allowed to have a sex life. It means the tablet manages the symptom while the cause continues, and if the cause is metabolic there is a treatment that addresses both. ED Can Be an Early Cardiac Warning
The arteries supplying the penis are narrower than the coronary arteries. Endothelial disease therefore tends to show up there first. New erectile dysfunction is a recognised early marker of cardiovascular disease, often preceding a cardiac event by several years.
This is the single most important thing on the page. A man who buys a compounded ED product from a website, and whose symptom then resolves, may have quietly bypassed the cardiovascular assessment his symptom warranted. The problem does not go away because the erection came back.
If your ED is new, get blood pressure, lipids, HbA1c and testosterone checked by a clinician who is not selling you the treatment. That is a cheap set of tests and it is the difference between managing a symptom and finding out why you have it.
Pricing
From $119 a month, with a doctor consultation included and rush shipping advertised as free. Set against the alternative, that is the weakest part of the commercial case: generic sildenafil and generic tadalafil are among the cheapest prescription drugs available in the US, frequently under $20 a month, and both are FDA-approved finished products with known doses.
So the question is what the premium buys. Convenience and a consultation, genuinely. A novel combination with clinical evidence behind it — no. There is no trial showing the stack outperforms an adequately dosed single agent.

What We Would Do Instead
Start with a single agent. Generic sildenafil or tadalafil, properly dosed, is cheap, FDA-approved, and works for the large majority of men. If one does not suit you, the standard next step is to try the other or adjust the dose — not to take all of them at once.
Get the workup. New ED warrants blood pressure, lipids, HbA1c and testosterone, because of what it can be signalling.
Treat the cause if it is metabolic. If you carry excess weight, that is very likely part of the mechanism, and the evidence above suggests treating it does more for erectile function than a fourth ingredient will.
If you use QUAD anyway, get the exact formulation and doses in writing, check them with a pharmacist against everything else you take, and never combine it with nitrates.

There is no evidence that it is. All three are PDE5 inhibitors acting on the same enzyme, so combining them adds dose rather than a new mechanism, while layering a short-acting peak on a long tadalafil tail and adding to the blood-pressure effect.

“Four-in-one” is a marketing number. Three of the reported ingredients share a single mechanism, sources disagree on the formula, and cheap approved generics do the same job with known doses.
More ingredients is not more medicine when the ingredients do the same thing. What the stack reliably adds is cost, overlapping duration and a compounded product whose contents independent sources cannot agree on.
The two things genuinely worth taking away: new ED can be an early cardiovascular signal and deserves a workup rather than a subscription, and if excess weight is part of the picture, the evidence now suggests treating that beats adding a fourth ingredient — in one head-to-head, it beat testosterone too.
