Berberine has the strongest randomized trial literature of any ingredient in the GLP-1 supplement space — and the most significant drug interactions. Both of those facts get less attention than the "nature's Ozempic" nickname, which is itself inaccurate. This page covers all three.
What Berberine Is
Berberine is an alkaloid compound found in several plants including goldenseal, barberry, Oregon grape, and Chinese goldthread. It has a long history of traditional use and, unlike most botanicals in the supplement market, a substantial modern clinical literature.
Its most-studied mechanism is activation of AMP-activated protein kinase (AMPK), an enzyme that acts as a cellular energy sensor and influences glucose and lipid metabolism.
Why It Is Not "Nature's Ozempic"
The comparison fails on mechanism. Semaglutide binds and activates GLP-1 receptors. Berberine acts on AMPK signalling. These are unrelated pathways producing different effects at different magnitudes.
If a comparison to a pharmaceutical is wanted at all, metformin is the closer analogue — both influence AMPK and both act primarily on glucose metabolism. Even that comparison overstates things: metformin has decades of large-scale outcome data behind it.

What the Trials Show
Berberine has been studied in numerous randomized controlled trials, and several meta-analyses have pooled them. The reasonably consistent findings are:
- Fasting blood glucose — reductions reported across multiple pooled analyses
- HbA1c — reductions reported, indicating an effect on longer-term glycemic control
- Lipids — reductions in total cholesterol, LDL cholesterol and triglycerides
- Body weight — smaller and less consistent effects than the glucose and lipid findings
That is a genuinely more substantial evidence base than the prebiotic and probiotic ingredients elsewhere in this category can point to.
Limitations of That Evidence
- Trial size and duration. Many constituent studies are small and run for weeks to a few months, not years.
- Methodological quality varies. Reviewers have repeatedly noted variable quality and risk of bias among the available trials.
- Heterogeneity is high. Studies differ in dose, formulation, population and outcome measures, which complicates pooling.
- Markers are not outcomes. Improving HbA1c is meaningful, but demonstrating reduced long-term complications requires far longer trials that have not been done for berberine.
- Formulation matters and varies. Given poor bioavailability, results with one preparation may not transfer to another.
- Weight loss is the weakest claim. If weight is your goal, this is not the ingredient with the strongest support for it.
Doses and Bioavailability
Trials commonly use around 500 mg two to three times daily with meals, so roughly 1,000–1,500 mg per day. Splitting doses is typical, partly for tolerability and partly because of berberine's short half-life.
Berberine is poorly absorbed orally — much of a dose never reaches systemic circulation. This is why some products use phytosome or other delivery technologies intended to improve absorption. When comparing products, look at the total daily amount and the formulation, not just the number on the front of the bottle.
Gastrointestinal side effects — cramping, diarrhoea, constipation — are the most commonly reported issue, and are part of why dosing is usually split.
Interactions and Safety
This is the section that matters most, and the one supplement marketing tends to omit.
- Cytochrome P450 inhibition. Berberine inhibits several CYP enzymes responsible for metabolising a large number of prescription drugs. Inhibiting them can raise blood levels of those medications — potentially into a harmful range.
- Additive glucose lowering. Combined with metformin, sulfonylureas or insulin, berberine can increase the risk of hypoglycemia.
- Pregnancy and breastfeeding. Berberine should be avoided; there are specific concerns regarding infants.
- Liver disease. Seek medical advice before use.
